Description:
Hyoscine butylbromide injection BP 20mg (commonly known as Buscopan injection or generic equivalents) is an antispasmodic medication used to relieve smooth muscle spasms and associated pain in the gastrointestinal tract, biliary system, urinary tract, or female reproductive organs. It is indicated for acute spasms such as renal colic, biliary colic, gastrointestinal cramps, irritable bowel syndrome exacerbations, and to reduce spasm during diagnostic procedures (e.g., radiology, endoscopy).
Prescription:
- Prescription required; administered only by healthcare professionals (intramuscular, intravenous, or subcutaneous injection).
Therapeutic Category:
- Antispasmodic / Anticholinergic (quaternary ammonium derivative; smooth muscle relaxant without significant CNS effects due to poor blood-brain barrier penetration).
Active Ingredients/Composition:
- Hyoscine-N-butylbromide (also called scopolamine butylbromide) 20 mg per 1 mL ampoule. Other ingredients: Sodium chloride (for isotonicity), hydrochloric acid/sodium hydroxide (pH adjustment), and water for injection.
Variant:
- Standard 20 mg/mL (20 mg/1 mL) solution in ampoules; some markets have 20 mg/2 mL or higher concentrations, but 20 mg/1 mL is the most common BP formulation.
Consume Type:
- Parenteral (intramuscular deep injection preferred; intravenous slow injection or infusion; subcutaneous possible).
Directions/Usage:
Administered by a doctor/nurse only.
- Adults: 20 mg (1 ampoule) by intramuscular, intravenous (slow injection 1 mL/min), or subcutaneous route; may repeat after 30 minutes if needed.
- Maximum daily dose: 100 mg (5 ampoules).
- Children: Not routinely recommended; dose if prescribed is weight-based (e.g., 0.3–0.6 mg/kg). No dilution required for IM/SC; for IV may dilute in glucose 5% or saline 0.9% if needed. Use for acute relief only—switch to oral/tablet form when possible.
Common Side Effects:
- Dry mouth, blurred vision, tachycardia (rapid heart rate), urinary retention, constipation, dizziness, flushing, or injection site pain. Rare: Allergic reactions (rash, anaphylaxis), hypotension, or confusion (minimal CNS effects compared to scopolamine).
Package Type:
- Clear glass ampoule (1 mL containing 20 mg); packed in boxes of 5, 10, or 25 ampoules.
Storage Advice:
- Store below 30°C, protected from light. Do not freeze. Keep in original carton. Use immediately after opening (single-dose ampoule). Keep out of reach of children.
Safety Advice:
- Contraindicated in myasthenia gravis, untreated narrow-angle glaucoma, prostatic hypertrophy with urinary retention, mechanical GI obstruction, megacolon, tachycardia, angina, cardiac failure, or hypersensitivity. Use caution in elderly, pregnant women (Category C—use only if essential), breastfeeding, or patients with cardiac arrhythmias. Avoid IM injection in patients on anticoagulants (risk of hematoma—use IV/SC instead). Monitor for anticholinergic effects (e.g., urinary retention, dry mouth). Report severe dry mouth, rapid heartbeat, confusion, or allergic reactions immediately. Not for long-term use—short-term relief only.
Product Substitutes:
- Buscopan Injection 20 mg/mL (original brand by Sanofi/Boehringer Ingelheim)
- Generic Hyoscine-N-Butylbromide Injection 20 mg/1 mL (various importers in Ghana)
- Spasmolyt or Scopolan injection equivalents
- Other antispasmodics (e.g., dicyclomine injection, atropine) if substituted by doctor. Consult a physician/pharmacist for available alternatives in Ghana.
