Description:Nebilet is a brand name for nebivolol, a selective β₁-adrenergic receptor blocker (beta-blocker) with additional vasodilatory properties. It is used to treat hypertension (high blood pressure) and, in some patients, mild to moderate chronic heart failure (particularly in older patients) as adjunct therapy. 

 

Prescription:Prescription-only medicine (POM in many jurisdictions) — must be prescribed by a physician.

Active Ingredients/Composition:

Consume Type:Oral tablet, to be swallowed (with water). 

 

  Directions/Usage:

  •  Usual dose for hypertension is 5 mg once daily (taken at the same time every day). 
  •  May be increased gradually by physician if needed (often every 2 weeks) up to a maximum (e.g. 40 mg for nebivolol in some regimens) 
  •  Can be taken with or without food. 
  •  In heart failure (especially in older patients), lower starting doses and careful titration are used. 
  •  Do not stop abruptly; tapering may be required under supervision. 

 

Common Side Effects:

Headache, dizziness, fatigue/tiredness 

  •  Swelling (peripheral edema) 
  •  Slow heart rate (bradycardia) 
  •  Gastrointestinal symptoms: nausea, diarrhea, abdominal discomfort 
  •  Respiratory effects in predisposed individuals (cough, shortness of breath) in sensitive patients with lung disease
  •  Possible masking of hypoglycemia signs in diabetics
  •  Less commonly, sexual dysfunction, sleep disturbances, etc.
  •  Rare but serious: hypotension, heart block, bronchospasm in susceptible individuals, exacerbation of heart failure in decompensated states

Storage Advice:Store at room temperature, protected from moisture and light. Keep in its original container. Do not freeze. Keep out of reach of children. 

 

Safety Advice/Precautions: Contraindicated in severe bradycardia, second/third degree AV block (without pacemaker), cardiogenic shock, decompensated heart failure, sick sinus syndrome (unless pacemaker), severe hepatic impairment. 

  •  Use caution in asthma or obstructive lung disease (may precipitate bronchospasm) 
  •  Use cautiously with conduction abnormalities or use of other heart rate-lowering drugs
  •  Monitor heart rate, blood pressure, renal and hepatic function during therapy
  •  In diabetics, be aware that beta blockers can mask signs of hypoglycemia (e.g. tachycardia) 
  •  Avoid abrupt discontinuation — taper gradually to prevent rebound hypertension or ischemic events
  •  Use in pregnancy only if benefit justifies risk; avoid or closely monitor in breastfeeding (< safety data)
  •  Interaction caution: many drug interactions, especially with CYP2D6 substrates/inhibitors
  •  Contains lactose — patients with lactose intolerance should consider that.
